Planning a visit to the United States? Want to

It’s easy to apply:
STEP 1:
Print the
application form (details on how to
apply are found on the form or click
here to download the
instruction sheet)
Complete the form in full and sign it.
Please ensure that you initial your acceptance or
rejection of Uninsured Motorists and/or
Underinsured Motorists coverage.
Provide a recent photograph of the motorcycle that
represents the condition and style of motorcycle.
STEP 2:
Choose method of payment as shown on application
form.
Add $60 policy fee to the calculated premium.
Please note US Dollar currency only.
Credit Card users will be charged a credit card
fee of 2.5%.
STEP 3:
Air Mail all documents, at least 45 days prior to
your trip, to:

If you have less than 45 days prior to
your trip, you may fax your application form to
407-937-1855 to begin the process. Please provide
an email address and a return fax number.
If
you require coverage for longer than 3 months, complete
this application for 3 months' coverage. Upon your arrival
in the United States, contact us at 1-800-391-8144, and
we will make the necessary arrangements to extend your insurance.

In the event of a claim:
Any claim against the policy must be reported immediately
to Dairyland Insurance at 1-800-638-8763,
in order to arrange an inspection prior to the motorcycle
leaving the United States.
Every attempt will be made to repair the vehicle prior to
departure.
You will need to provide Dairyland’s Claims Department with
information on how to contact you while you are traveling
abroad.
